PPO insurance accepted upon approval. To find out what your cost would be, call the number on the back of your insurance card. Ask the following questions:

What is my outpatient mental health coverage for "out of network" providers? They will most likely give you a percentage covered.

What is my "out of network" deductible? Have I met my deductible? If not, how much is left? Is this renewed per calendar year or month insurance became active?
